Our children attend a variety of secondary schools. Our cluster has appointed a transition worker to support vulnerable children (including SEND) in their transition to Tanfield School. However, we work closely with all of our local secondary schools to support transition of any children with SEND. Part of the role on the SENDCo is to liaise with the next school and share information regarding SEND.
Children with SEND are given an extensive level of support before they leave for a new school. Their teacher and SENDCo will discuss with secondary colleagues how many transition visits can be arranged over and above those offered to children without SEND. These additional visits to the new school will be carried out with the child and Secondary School Staff in the summer term or sooner if necessary. This ensures that bridges are built between schools, key information is shared between staff and parents, and the child is given the opportunity to settle into a new routine with the support of a familiar adult. Where appropriate, external agencies will also support the transition program.
We also support the transition between the various key stages in our school for children with SEND. This may involve additional visits to the new teacher and familiarisation with routines and layout of the building. This is usually carried out with our support staff in the summer term on an individual basis. Where appropriate, children may be given a transition book which is shared with the parents so that they can continue this process of preparation over the summer break.
